  ** NOTE: Use to register both initial shot and any booster shots

FORMS FOR OUTSIDE SESSION INTERACTION

• Meeting with a Student:  If you want to meet with a student in-person outside of Minds Matter Saturday Sessions the following form must be completed and approved (Parent/Guardian permission):

** NOTE: You must meet in a public place in the presence of other adults

• Driving a Student: If you want the ability to drive a student - the following two forms must be completed and approved (Parent/Guardian permission and DMV/Insurance check):

     2. Minds Matter DMV Check Form - Link HERE 

** NOTE: To be cleared to drive, a DMV check must be complete and you must submit proof of current insurance

** NOTE: There must be at least 2 adults in the vehicle.
