  ** NOTE: These policies are to be read and signed off at the start of volunteering.

FORMS FOR OUTSIDE SESSION INTERACTION

• Meeting with a Student:  If you want to meet with a student in-person outside of Minds Matter Saturday Sessions the following form must be completed and approved (Parent/Guardian permission):

** NOTE: You must meet in a public place in the presence of another Minds Matter-vetted adult

• Driving a Student: If you want the ability to drive a student - the following two forms must be completed and approved (Parent/Guardian permission and DMV/Insurance check):

  1. 1. MM PDX Request to Meet Outside Session with Student - Link HERE

     2. Minds Matter DMV Check Form - Link HERE 

** NOTE: To be cleared to drive, a DMV check must be complete and you must submit proof of current insurance

** NOTE: There must be at least 2 Minds Matter-vetted adults in the vehicle.
